Assertive community treatment

A team-based clinical approach that involves multiple disciplines, which helps individuals with severe mental illness to reside in the community by offering 24-hour intensive community services in their natural environment.

Diet and exercise counseling

Easterseals Blake Foundation may offer information and support (such as training in skills and access to resources) to individuals with an emphasis on the relationship between physical and mental health. Family psychoeducation

ICM evolved from Assertive Community Treatment (ACT) and Case Management (CM) and emphasizes frequent contact, small caseloads, and high-intensity care to improve the consumer's quality of life. The case manager coordinates services from multiple systems as the consumer's needs change, working to help them achieve their recovery goals and stabilize their lives. Intensive case management is critical in providing the support and resources that consumers need to improve their quality of life.

Case management service

A service that provides assistance to individuals and families in coordinating necessary support and resources through a designated case manager. The case manager assesses the needs of clients and their families and arranges for various services such as mental health care, social work, healthcare, education, employment training, recreational activities, transportation, advocacy, and respite care as needed.

Suicide prevention services

Involve identifying potential warning signs; educating staff on recognizing signs of suicidal behavior and evaluating risk; and managing patients who are at risk of suicide by providing appropriate interventions, such as treating underlying mental health or substance use disorders, administering psychotropic medications, offering supportive services, and providing education.
